Reporting Income
If you are eligible to receive Ontario Works financial assistance, you will
want to make sure your cheque arrives on time each month.

Be sure to submit your
statement of income card on time. Return the card on the 16th of each month.
Do not return the income statement before the 16th of each month
or it will be returned to you.

You must report all income
received.

If you do not have the income
listed you must record NIL in the box.

Remember to sign and date the
income statement on the back.

Be sure to attach pay stubs
or other verification such as babysitting receipts, if you are reporting
earnings and/or other income.

Your income statement must be
completed and returned every month, whether you have income or not. Benefits
are suspended if it is not returned.
Remember that any changes in your circumstances can affect your
assistance and, in order to protect your own interest, it is important to let
your Case Manager know as soon as possible.

Assets
You must report any assets you own. An asset is anything that can be
readily converted into cash for your benefit, or for the benefit of your
spouse or children. Assets include but are not limited to the following
examples:
